Parental Consent Form

On July 18, 2020 the FA issued guidelines for the return of outdoor competitive football in accordance with the Government’s approval of their plan.  In order to comply with the FA rules and guidance, Jets FC is implementing a number of changes to procedure to ensure that we adhere to the rules in order to operate safely.  The following is a summary of the more significant rules, however, you may read the complete rules and guidance by clicking here


  • Everyone must self-assess for Covid-19 symptoms before and after every training session and/or match.  Do not leave home if you or someone you live with has any of the following: a high temperature, a new continuous cough, a loss of, or change to your sense of smell or taste.  In this situation you must stay at home and follow the latest Government guidance.
  • Although football activities are resuming, other preventative measures are sill in force and you must follow Government guidance for travel, including minimising use of public transport.
  • Personal hygiene is important with everyone should bring their own personal hand sanitiser and water/drinks, clearly named, for use by by the players at training sessions or matches.
  • Changing facilities are considered a high-risk environment, therefore, players should arrive fully dressed in match or training kit.
  • A register of all football activity attendees will be completed by the relevant team manager/coach.  This is to support the NHS track and trace process if subsequent infection is reported and the club will hold this data on file for 21 days after the activity.
  • For players under 16 we require the parent/guardian/carer to remain present at all times during the sessions or matches (following social distancing guidelines) to assist should their child require assistance, e.g. first aid from an injury.
  •  All spectators must continue to observe the social distancing requirements and cooperate fully with club officials should they require assistance.
  • In addition to the club’s Code of Conduct, chewing gum and spitting are prohibited.  Additionally, smoking and vaping is also not permitted.
  • Goal celebrations in close proximity to other players is strongly discouraged until further notice.
  • Handling of balls should be kept to a minimum.
  • Hands should be sanitised/washed regularly during and after any football activity.
  • The club must be informed immediately if anyone subsequently develops symptoms within 14 days of any of our activities.
  • Any child/player who has been diagnosed with Covid-19 must have proper medical clearance from their doctor before rejoining any of our football activities. 

As part of the new guidance from the FA, parents must indicate that they have read the guidance in full and give written consent for their child to attend any football activity.  For children/players under the age of 16 this consent must be provided by the child’s parent, guardian or carer.  The club is making two options available for the written consent to be given.  Parents/guardians/carers have the option of signing a written template statement which will be provided by a club official (respective team manager, coach, or club secretary), or they can send an e-mail to
